Personalised care is central to the NHS’s ambition to deliver more person-centred and sustainable care. Through the NHS England Comprehensive Model for Personalised Care, the NHS has developed approaches such as shared decision making, personalised care planning, social prescribing and personal health budgets to support people to take a more active role in managing their health and wellbeing. As demand continues to grow and more people live with multiple long-term conditions, delivering personalised care across the NHS will be important in reducing health inequalities and supporting the shift from hospital to community-based care.
